Which size do I get from a multi-size ICO?

Whichever the browser's decoder returns, typically the largest packed inside. To pick a specific size — particularly one that was drawn separately rather than scaled — use a dedicated icon editor.

Is transparency preserved?

Yes. Icons almost always have transparent backgrounds and PNG carries the alpha channel exactly.

Why would I convert an icon to PNG?

To edit it, to reuse the artwork somewhere else, or because a tool or document will not accept an .ico file.

Can I convert it back afterwards?

You can produce an .ico for use as a website favicon. A true multi-resolution ICO for Windows application icons needs a dedicated tool — see the PNG to ICO page for why.
