Could I just rename the file to .mp4?

Often, yes — the contents really are an MP4. A proper repack additionally rewrites the index and sets the faststart flag, which matters for playing the file over a network while it downloads.

My M4V will not convert — why?

Almost certainly FairPlay DRM, which iTunes Store purchases and rentals carry. Encrypted files cannot be read by anything that respects the encryption. Files you made yourself convert normally.

Is quality lost?

None. The video and audio are copied without being decoded, so the result is identical to the source.

What is actually different between M4V and MP4?

Very little beyond the extension and Apple's use of it to signal iTunes content. Some M4V files also use Apple-specific flags, which a repack normalises.
